Manager Long Term Care – Palliative Medicine, Spiritual Care

In this role, you will manage programs and activities which may include DME, SNF, eldercare, palliative care, and other continuum programs, as assigned. You will be responsible for patient safety and regulatory compliance while ensuring the Kaiser Permanente Health Plan members receive appropriate, focused, quality care.

Qualifications Include:
• At least three years of experience in a skilled nursing facility for the SNF, DME, and eldercare program work, or one year of experience in a palliative care related job required for the palliative care program work
• A bachelor’s degree; a master’s degree preferred
• A current California RN license preferred
• Demonstrated management and leadership skills
• Knowledge of SNF, Medicare, The Joint Commission, and other federal, state, and local regulations
• Prior knowledge and experience with palliative care, hospice, transdisciplinary teams and/or advance care planning is preferred
• Thorough knowledge of Kaiser Health Plan benefits
• The ability to work in a Labor/Management Partnership environment
