Job Summary:
The person in this position is responsible for the fostering palliative cultural change and coordinating/implementing palliative consults within the hospital

Essential Generic Job Functions (List in order of importance):
Coordinate and implement inpatient consultation services in palliative care.
Facilitate clarification of patient and family goals of care.
Consult with the attending physician and the Interdisciplinary Team to establish a written plan of care at intervals specified in the plan.
Facilitate access to appropriate supportive care services.
Facilitate family meetings.
Facilitate outpatient palliative care service in Dept Pain & Palliative Medicine.
Full and appropriate documentation of patient care services to support professional billing for these services.
Education of patients, families and community members
Champion Program, Palliative Mentor Program etc.
Collaborates with short and long term care facility liaisons to educate current and potential referral sources.
Participation in on-call coverage with the Dept Pain & Palliative Medicine.
Weekly participation in interdisciplinary team meetings to review active cases.
Assist in the development of standards of care.
Adhere to the practice of confidentiality regarding patients, families, staff, and the organization.
Conduct and participate with palliative research projects when approved by the IRB.
Adhere to Joint Commission Standards as related to DSC for Advanced Palliative Care
Education of professional stakeholders; lunch and learns, Grand Rounds, annual conferences, Pain

Knowledge, Education and Skills Required:
Master of Nursing in Adult or Family Nursing Practice
Graduate level Palliative or End of Life Care Education
Board certified in Adult or Family Nursing Practice
Board certified in Palliative Medicine within 1 year of hire
Active and in good standing NJ state APN license
2 years of palliative care clinical experience
Excellent communication skill and interpersonal skills
Demonstrated knowledge about home care, hospice, palliative care, advance 
care planning, pain management, symptom control, and discharge planning
