Position Summary:
Responsible for the leadership, development, coordination and
management of Hawaii Care Choices clinical services and the direct delivery
of care for patients and families. Plays an essential role in achieving our
mission of providing people and families facing serious illness, death and
grief the highest quality end-of-life care and support.

Duties & Responsibilities:
o Direct supervision of direct patient care clinical staff.
o Provide guidance, support and coaching on staff development and
performance expectations for all those under your supervision.
o Explore, develop and implement hospice and palliative care industry
best practice methods/models of care to assure the availability and
provision of appropriate care and services across all service lines.
Including but not limited to staffing patterns, clinical staff
productivity, competencies based on and that meet industry standards,
and evidence-based practice.
o In collaboration with the Clinical Operations Manager, develop and/or
revise policies, procedures and guidelines to fulfill program objectives.
o In collaboration with the Clinical Operations Manager, evaluates
staffing requirements and recommends changes in the staffing
patterns to ensure staffing meets demands of referral-to-admission
conversion ratios. Consistently uphold and enforce established
standards of excellence fostering responsibility and accountability
within each team member

Skill & Knowledge Requirements:
o Clinical skills competence with the ability to mentor others.
o Sound working knowledge of hospice and palliative care Conditions of
Participation (COPs) and Clinical Practice
o Demonstrated organizational and critical thinking/analytical skills,
detail-orientated, and follow up.
o Strong leadership, interpersonal and communication skills
o Ability to adapt to changing situations on short notice and with ease
o Understanding and acceptance of hospice and palliative care
o Advanced and efficient use of technology.
o Working knowledge of EEOC regulations & risk management, as they
pertain to health care employees.
Education & Experience Requirements
o BSN required, MSN preferred from an accredited program and
licensed in the State of Hawai`i.
o Two (2) years hospice management and Five (5) years leadership
experience preferred.
