Office-Based Palliative Care Practices: Strategies for Success

  • Wednesday, September 18

    1:30 p.m. - 2:30 p.m. ET

Office-based palliative care programs are increasingly in demand as organizations realize that the historical model of providing palliative care to hospitalized patients may miss a critical point earlier in the disease trajectory. Studies show that palliative care is associated with reduced health care utilization and improved survival rates. It is time to expand access to palliative care by reaching patients with serious illnesses proactively in an office-based setting, rather than reactively within an acute care setting.

Despite the widespread movement to create earlier access to palliative care, many organizations struggle to successfully design and grow an office-based palliative care practice. Palliative care programs are under increasing pressure to implement a new practice quickly, which can strain resources when planning is not done strategically. This webinar will provide practical tips and outline the steps necessary to create a sustainable, scalable office-based palliative care program, including how to:

  • Obtain buy-in from key stakeholders and conduct an initial needs assessment
  • Determine the target patient population and scope of practice
  • Identify and measure program evaluation metrics
  • Plan for growth while preventing burnout
