Bridging Gaps in the Continuum of Care through Local Partnerships

In this webinar, the presenters will describe their program’s method for identifying and bridging gaps in care for people with serious illness, focusing on the process for partnering with other agencies to provide service across the continuum of care in order to efficiently care for patients in a shared-risk environment. The method for identifying common gaps in care, including medication issues, crisis management, caregiver stress, social isolation, care coordination and economic barriers, for their Medicare Advantage community will be addressed, along with the structure of the program, including protocols and workflows implemented to support the program. Attendees will walk away with lessons learned from this program’s model and experience that will help them learn how to create a program to identify and develop partnerships with local community partners and implement a program that can facilitate 24/7 care and reduce unnecessary ED admissions and hospitalizations.
