“Early Bird” rate of $900 (individual) or $800 (special rate*) will be available through Friday, October 3.
Fee after October 3 is $1,100 (individual) or $900 (special rate*).
Program registration fee includes a copy of A Guide to Building a Hospital-Based Palliative Care Program; Friday evening Poster Session and Reception; breakfast, lunch and coffee breaks each day; and educational materials.

CANCELLATION POLICY
A $100 administration fee will apply to all cancellations. A full refund, less the administration fee, will be made for cancellations submitted on or before October 28. No refunds will be made on cancellations received after this date. All cancellation or substitution requests must be submitted in writing, via fax (212.426.1369) or e-mail (capcevents@mssm.edu). Please allow 30-60 days for processing refunds. Substitutions are welcome.
