In order to receive continuing education credits for the September 15 Audioconference, the following conditions must be met:

• For each site (i.e. each participating phone line) a Site Coordinator must read the Site Coordinator Instruction Sheet and put out the attached sign-in sheet.
• The Site Coordinator must ensure that anyone whoe wishes to recieve CE credit sign their name on the sign-in sheet provided by the site coordinator.
• After the call, all participants must fill out an evaluation form.
• Nurses who wish to receive credit must read and complete the Nursing CE Request Form.
• Social Workers who wish to receive credit must read and complete the Social Worker CE Request Form.

All information regarding fees and where completed forms should be sent is on the appropriate form.
