About the Presentation
Palliative care social workers face increasing demands to identify patients and families most in need of specialist-level psychosocial intervention. At the same time, there are greater opportunities for formal and informal social work peer education on issues pertaining to care of the seriously ill. This session will help palliative care social workers and their colleagues to identify several models of collaboration, triage and education for times when team census is high, as well as strategies for effective case finding and outreach when census is low.

Learning Objectives
- Distinguish between specialist and generalist social work skill sets in palliative care, identifying those triggers that highlight the need for specialist-level intervention
- Describe several forms of collaboration and/or triage between palliative care social work and unit– or team-based social workers
- Identify several models of peer education on issues pertaining to psychosocial aspects of palliative care, opportunities for case finding and outreach afforded by the palliative care social worker’s unique skill set
