About the Presentation
Growing numbers of the frail elderly, including many with multiple co-morbidities, are residing in long term care settings today. In fact, 66% of them continue to reside there until they die–representing roughly 25% of all deaths. This presentation will discuss several models for increasing access to palliative care within long term care settings and addresses some of the barriers that currently exist.
