This report, and the study upon which it was based, were made possible by grants from:

Archstone Foundation, Brookdale Foundation, Donaghue Foundation, the Emily Davie and Joseph S. Kornfeld Foundation, the John A. Hartford Foundation, Mill Park Foundation, the Olive Branch Foundation, Partnership for Palliative Care, the Robert Wood Johnson Foundation, the United Hospital Fund and the Y. C. The major findings contained in this report build on research first described in “Variability in access to hospital palliative care in the United States” (Journal of Palliative Medicine, October 2008). The data were obtained primarily from the American Hospital Association (AHA) Annual Survey DatabaseTM for fiscal year 2009. Additional data for the 2011 report were obtained from the American Board of Medical Specialties (ABMS), American Academy of Hospice and Palliative Medicine (AAHPM), National Board for Certification of Hospice and Palliative Nurses and the Dartmouth Atlas.

The Center to Advance Palliative Care (CAPC) provides health care professionals with the tools, training and technical assistance necessary to start and sustain successful palliative care programs in hospitals and other health care settings. Located at Icahn School of Medicine at Mount Sinai in New York City, CAPC is a national organization dedicated to increasing the availability of quality palliative care services for people facing serious, complex illness.
