Clinical Site Visit Directory

The Center to Advance Palliative Care (CAPC) receives frequent requests for information regarding palliative care teams willing to host clinicians interested in on-site clinical training. In response, CAPC has partnered with the American Academy of Hospice and Palliative Medicine (AAHPM) to develop a national Clinical Site Visit Directory.

Offerings for clinical training vary by institution.

Curriculum Description:
The UAB Clinical Training Academy includes a tailored combination of morning didactic teaching sessions with faculty/staff coupled with direct observation of patient care on inpatient palliative and comfort care unit, a palliative care consultation service in a large acute care hospital, and daily debriefing with a primary provider mentor. Standard topics include Introduction to Palliative Care, Communication Skills, Complex Care Coordination, Interdisciplinary Team Dynamics, and Pain & Symptom Management. Specific individualized time and curricular content for participants interested in ambulatory care settings can be provided as well.

Learning Experience Offered:
Emphasis is placed on direct observation and participation with an established acute palliative care team which includes all other disciplines in which a physician would encounter in a full coverage palliative care setting. The CTA experience places the physician learner in the settings of both an established palliative and supportive care unit as well as a high volume consultation service that covers over 900 acute care beds in every section of the hospital.

Learning Experience Offered:
Visiting clinicians may attend daily patient discussions, observe and discuss consultations, and participate in fellow didactics, journal club and case discussions. Visitors are offered the opportunity to round with members of the team, including physicians, an advanced practice nurse, chaplain and clinical social worker. Additional experience in a large community hospital and with the palliative home care program may be possible.

Interactive group and individual case and topic discussions plus clinical observation cases consulted on by teams both in outpatient and inpatient services included.

Curriculum Description:
Our well-established clinical program features a dedicated 12-bed inpatient palliative care unit, as well as consult requests which originate in virtually every unit in our 750-bed tertiary care hospital, including the Emergency Department.

Our weekday operations include two teams of board-certified attending physicians and NPs accompanied by resident and medical student trainees who round on approximately 25 to 30 hospitalized adult patients each day. We also have a strong research arm.

Curriculum Description:
Physicians and nurses will have the opportunity to gain exposure to inpatient consultative palliative care delivered by a well-established program within a major academic medical center. Opportunities also exist to observe in our large ambulatory palliative care practice and our community-based inpatient hospice unit.

During the week, observers will have the opportunity to ask questions about management issues, observe communication techniques and attend educational sessions. Ongoing long distance mentoring may be individually determined.

Curriculum Description:
The Baylor University Medical Center Supportive and Palliative Care team offers training in the major domains of palliative medicine including the following aspects of care:

  5. Imminently dying patient
  6. Ethical and legal

Learning Experience Offered:
The trainee is given the opportunity to structure a learning experience designed to meet his or her unique needs based upon prior experience and personal learning objectives, by participating in daily huddle, weekly team meetings, new consults and daily rounds (average daily census 20 patients). They may chose to spend time working with board certified palliative medicine physicians and/or nurses and nurse practitioners, but will also have access to work with pastoral care, complimentary therapy providers, and others if desired.

For the trainee with an interest in clinical ethics, there will be opportunities to participate in our active clinical ethics consultation process as well (8-10 consults per month). This all takes place on a 1000-bed teaching hospital campus.

The Virginia Initiative for Palliative Care, co-developed by VCU Massey and Capital Hospice of Falls Church, Virginia, provides highly individualized training to doctors, nurses, chaplains, social workers and other professionals who seek tangible ways to improve patient care.

The cost is $1,000 for physicians and $500 for other disciplines.

Learning Experience Offered:
Clinical training offering an observership experience. Participants are exposed to palliative care in settings that are similar to their own environment with the goal of understanding how to improve the clinical experience and learning when to call for help. This can include clinic, consults or inpatient unit.
