Detroit Receiving Hospital's Palliative Care Service, Detroit, MI
Pioneered a proactive, interdisciplinary palliative care consultation service in the ICU in the 1980s and has continued as a model program in this field, with multiple peer-reviewed reports of the successful outcomes it has achieved.

Pediatric Advanced Care Team (PACT) at the Children's Hospital of Philadelphia (CHOP), Philadelphia, PA
This interdisciplinary team under the direction of Tammy Kang, MD, received the American Hospital Association's 2008 Circle of Life Award for programs that "expand the frontiers of palliative and end-of-life care." Veteran's Integrated Service Network 3 ICU Palliative Care Initiative
VISN 3 in the New York-New Jersey region of the Department of Veterans Affairs Veterans Healthcare Network has broken new ground with its VISN-wide ICU palliative care initiative in five medical intensive care units. This initiative has developed several innovative tools to facilitate timely and effective ICU family meetings. VISN 3 is also pioneering a communication skills training laboratory to equip ICU nurses for active participation in interdisciplinary meetings with ICU families.
