New York State Health Foundation Grant

New York State hospitals, and hospices with an established hospital palliative care program partnership, are eligible to apply.

As part of its continuing effort to improve New York State's health care system, the New York State Health Foundation (NYSHealth) has awarded the Center to Advance Palliative Care (CAPC) a grant to help New York state hospitals develop, strengthen and grow strong palliative care programs. The project utilizes proven technical assistance from CAPC. The goal is to increase access to quality hospital palliative care in New York State. Space is limited. Apply today!

CAPC National Seminar:

The CAPC National Seminar provides essential operational training that ensures growth and sustainability for hospital palliative care programs at every stage. Each year the seminar offers new cutting-edge content and topics.

Palliative Care Leadership Centers™ Training and Mentoring

Palliative Care Leadership Centers™ (PCLC) provide intensive, hands-on operational training and yearlong mentoring. PCLC Custom and PCLC Consult are specifically customized for active programs, while PCLC Core is tailored to meet the needs of programs in the planning and start-up stages. PCLC Peds is for pediatric programs at every stage of development.

To apply:

  • You may enroll immediately via the online PCLC application.
  • All hospitals utilizing the NYS Health Foundation grant are strongly encouraged to register their palliative care program with the National Palliative Care Registry™ at https://registry.capc.org. If your program has already registered for this year, you will not need to register again.

Please note: Grant funding covers tuition costs only. Grantees are responsible for all other expenses including travel and hotel, or any additional costs for reverse trainings (trainings where PCLC faculty comes to your institution). Deadline to apply is June 31.

CAPC Audio Conferences:

CAPC offers audio conferences that support and enhance the development of palliative care programs in hospitals. Featured speakers are nationally recognized leaders in the field of palliative care.

To apply:

  • Select the “Register Now” link for the audio conference you want to attend.
  • Use coupon code: NYGrant
  • You will receive a registration confirmation via email within 24 hours.
  • Detailed instructions to access the audio conference will be emailed 48 hours prior to the start of the audio conference.
  • All hospitals utilizing the NYS Health Foundation grant are strongly encouraged to register their palliative care program with the National Palliative Care Registry™ at https://registry.capc.org. If your program has already registered for this year, you will not need to register again.

CAPC Campus Online™ e-Learning Courses:

CAPC Campus Online™ provides online CME/CE courses covering all the structural, operational and financial essentials of palliative care program development and sustainability. Our palliative care e-learning courses offer you complete flexibility because they are available when you are available, 24 hours a day, seven days a week.

To apply:

  • If you already have an account, log into your account and enroll in a course. You will not be asked for credit card information.
  • If you don′t already have an account, click “New Account” and follow the directions onscreen to create a unique “Username” and “Password” to access courses. Once you identify your hospital in our system, you will not be asked for credit card information.
  • When you return to CAPC Campus Online™ you must “Login” with your username and password at the top right of the screen and proceed to enroll in an e-learning course.
  • All hospitals utilizing the NYS Health Foundation grant are strongly encouraged to register their palliative care program with the National Palliative Care Registry™ at https://registry.capc.org. If your program has already registered for this year, you will not need to register again.
