CAPC Palliative Care Discussion Forum
Staffing
We don't have a specific tool to use for hand off but have some strategies that have proven helpful to make sure information get's passed on. Our staff prints a list of the patients on service and leaves pertinent information r/t the patients on that list. They also may give a verbal or e-mail report. The clinical record is also an excellent means of passing on information in progress notes and dictations. In our system these are all electronic and easily accessed by the on-coming team memebers. The use of protocols and or order sets also is a means of standardizing care that increases the likelihood of quality care being delivered across care providers. I would suggest having a conversation with your on-call team to see what method works well for them and then implement accordingly.
