Hi. Social Work may very well be an excellent choice. The social work role in palliative care encompasses individual and family counseling, advanced care care planning in conjunction with the other members of the team ,and psycho-education to enhance coping ,to name a few dimensions of the role and certainly includes aspects of coordination of care,too, as a practical matter. You may want to explore SW the job descriptions on the CAPC web site, and maybe check out the Smith Certificate program in Palliative and End-of-life Care to get an idea of the role and the tasks involved.
