Do you weight your patients?
Our nurses are expected to casemanage 28-30 patients for a 40 hour work week. Hospice patients are weighted at 2 (counting double a regular homecare patient). Right now Palliative Care patients are also weighted at 1 like a regular homecare patient, however they require much more intensive nursing interventions, etc. Does anyone out there use this system? And if so how do you count a Palliative Care Patient?

Susan I know program vary in weights for visits, and adults usually vary from peds.
However in our palliative care program for pediatric patient we weight a start of care or an admission visit as 2, and then any revisit as 1. If that revisit goes over 2 hrs it will be weighted differently. 1.5 to 2.0, sometimes 3 if it's very long; however this is not the norm unless it's at end stage. Most of our palliative care visits are one hour. Our nurses and social workers are held to a productivity of 4 patients/ 8 hrs.

In our adult program they are case managing 25 patients / week. In peds it's 20/ week for full time staff. Our peds nurses travel more.

Some computer programs for hospice and palliative care have recommendations for weighting patient visits but require that the organization using the program define the parameters for weighting. There are national bench marks for productivity including Fazzi that recommend a productivity of 3.5 visits for hospice and this factors in on-call, triage, admission visits, regular visits, etc. If you are looking at productivity for PC you will also want to factor in multiple components of the visiting nurses responsiblities.
