However in our palliative care program for pediatric patient we weight a start of care or an admission visit as 2, and then any revisit as 1. If that revisit goes over 2 hrs it will be weighted differently. 1.5 to 2.0, sometimes 3 if it's very long; however this is not the norm unless it's at end stage. Most of our palliative care visits are one hour. Our nurses and social workers are held to a productivity of 4 patients/ 8 hrs.
In our adult program they are case managing 25 patients / week. In peds it's 20/ week for full time staff. Our peds nurses travel more.
