Depending on the culture in your institution, many clinicians on the pc team may be able to respond to the PCP. However, in many places, physician-to-physician communication is the norm. Also, it makes sense to find out how PCPs whant to receive communication (call, fax, email, etc). For many issues, an email can work... but person-to-person communication is often key for big issues. Also, almost all PCPs appreciate a fax with the discharge summary/plan, including the updated med list.
