I am attempting to develop an APRN collaborative practice agreement and was hoping someone might have a template, or be willing to share.

Our NP's have a Standard of Care Arrangement for here in Ohio. I will share it with you per your e-mail. Please just remember that there are state laws that need to be considered in establishing an agreement.
