Our NP's have a Standard of Care Arrangement for here in Ohio. I will share it with you per your e-mail. Please just remember that there are state laws that need to be considered in establishing an agreement. Sharol - Could you please send me a copy of your collaborative care arrangement as well. I have a standard format that I received but has no discussion regarding palliative care.

I hope you're not being too barraged with requests for your standard care agreement, but they just aren't out there, and I could sure use a template for my new practice in newark. Thank you!

Thank you for acknowledging my request for a copy of a Standard of Care Arrangement that you are utilizing in Ohio. I fully understand the need to take in to consideration state laws for PA.
