We have a similar set up. I carry a cell phone and check it on off hours. Patients can leave a voice mail and I can triage. I feel the best way to keep crisis down is education. Prepare patients for the "what ifs" and I check in with fragile patients at the end of the day and Friday nights and sometimes call them on the weekends. But all the time doing self care too. If I go on vacation another person can carry my cell phone.
