Post criteria for social work consults

I am honored to be the first social worker within in an in-pt and out-pt palliative care program. Does anyone have a specific criteria listing for social work consults. Everyone can benefit but we need to streamline the consults to be more specific.

The social worker for the Palliative Care Consultation Team meets with the PCCT when we have a family meeting and decisions are made for what needs the patient and family has at that time.

The process:
Consultation made to the PCCT from any service/physician

The NP reviews the admission criteria to determine appropriateness of referral;

The NP evaluates the patient, and schedules a family meeting that includes the patient and those persons who need to be at the FM per patient and HCS's wishes

All PCCTeam Members are present at the FM and the needs are discussed, each member of the team then addresses the needs per their specialty.

We have a screening tool that we give to all doctors and nurses during their orientation. It lets them know when to refer to a Transitions Counselor/social worker and/or for the whole palliative team. Our Transitions staff can handle much of the psychosocial palliative issues without the need for the clinical staff. Most times it is a precurser to the palliative consult. You can find this on the CAPC website under Tools, called "palliative care screening tool" Medical Center of Central Georgia.
