The process:
Consultation made to the PCCT from any service/physician
The NP reviews the admission criteria to determine appropriateness of referral;
The NP evaluates the patient, and schedules a family meeting that includes the patient and those persons who need to be at the FM per patient and HCS's wishes
All PCCTeam Members are present at the FM and the needs are discussed, each member of the team then addresses the needs per their specialty.
