I have been using the distress thermometer along with a social worker to screen patients going home to rural Alaska for end of life care. I have found it to be a very useful tool and it often identifies things that have not been directly addressed during the initial intake. We added it to our intake toolbox but only administer it after initial meeting and establishing a good provider patient/family relationship.
