Hi. There is a tool that was developed for home based hospice care that may be helpful to look at. It is called the SWAT(social work assessment tool, and there is literature on it that was authored by Donna Reese et al, published a few years ago. The article includes a copy of the tool. The tool itself is available on the NHPCO website. In our inpt program, we use the standard of a SW clinical assessment/ narrative note that includes assessments of individual pt in family context, socio-economic situation, background/history, presenting concerns, strengths and vulnerabilities, resources and needs, etc.
