I am the palliative care coordinator for two small rural hospitals in one market of a larger system. I have been in this role for over 3 years and am also a LCSW. I currently work with a physician, but am working to expand the team to include a nurse. This may evolve into a social worker/nurse team that does primary palliative care assessments, utilizing a physician advisor for complex clinical cases.I am also happy to talk with others about their models of delivery and experiences.

I am the Director of the Palliative Program at a large hospital in Macon, Georgia. I am a Family Therapist with a Social Work background. We recently published an article in the Journal For Palliative Medicine titled, "A Novel Approach to Hospital Palliative Care: An Expanded Role for Counselors." Look for it in this months journal! We have advance practice nurses and palliative physicians, but have a strong focus on the psychosocial aspects of palliative care. We believe this is why we have been successful.
