Great question. Every palliative care service needs to identify and define its service standards for multiple reasons (this determines volume, scope, staffing and budget, among other variables - and the Joint Commission requires that we have this information formally established). Certainly the tool you are describing would be a perfect adjunct to this - a child- and family-specific guide to accessing what a program has to offer. I'm not aware of any published tools either -- I think most programs create individualized care plans and follow the standards laid out in the National Quality Forum. But what you are describing would be a great thing to collate and share on the CAPC tools page so that all of us could benefit from each other's thoughts and expertise. Perhaps you can start us off - and there may be others out there who can join and share.
