But as part of your admission should be goals of care and interventions for the patient and family. We currently have patients in our palliative care program who have been patients for years. They continue to be enrolled due to the continued roller coaster ride the patient experiences as well as life changes in the family unit. There have been others that have been graduated from the program - patient has a stable course now, family feels well equipped to care for the child, etc. So again although at time of admission, a diagnosis is an appropriate screening for palliative care supportive wrap around services, does the patient and family have unmet needs that being enrolled in such a program can address and then once met have graduation.
