I am obtaining the POLST during hospitalization put a note in the Problem List of the patient so it will show up on subsequent admissions that the parents have made advanced directive decisions. We do not scan in the form into the system. Each re-admission is a decision point with my pediatric patients, so it requires reconfirmation that info is still valid, then we transfer the info as an attending order into our EMR. The MD who originally signed it may not have priviledges at our center, so the attending must enter into EMR. I look at it the same as meds reconciliation.

Yes---we scan our PALI (Parent Approved Level of Intervention) forms into our EMR after they are discussed and signed by the MD/PNP and parent/guardian...

Yes, we scan the POLST form into the EMR and send the original with parents. We ask parents to bring the POLST back with every hospital visit and update as needed.
