CAPC Palliative Care Discussion Forum
Pediatric Palliative Care
Hi - Thanks for the question. The team covenant is a document that each team member signs to ensure there is a shared loyalty and understanding of what it means to be on the team. The team care plan reflects the individual care plan that each team member is asked to develop. The care plan has specific actitivities that help the team member tend to each of the four areas of spiritual, emotional, physical and psycho social. For example a team member may use meditation or prayer or time in nature to address thier own spiritual needs, identify two team members that they can go to for emotional support at work when they need it, Exercise or do yoga or relaxation for caring for them selves physically and have specific thing they do for fun or recreation for the psycho social area.
The team care plan needs to be more broad as there are so many different preferences on the team but may include things like taking time for reflection before team meetings, doing some things together socially, taking time to debreif and share confidentially when the hard cases come and having understanding about how to handle inter team conflict etc.
If you have more quesitons I would be happy to talk further about this. Feel free to contact me if you have more questions.
Thanks
