Hi. This is an important topic, and I will look forward to hearing about your research! In the meantime, I can say that our agency has been in the process of developing a network of community based hospice resources to collaborate with our hospital as we discharge kids to areas beyond our service boundary. These agencies are ina reas that range of small towns,to small cities to rural areas. The barriers and challenges we have heard most often have to do with 1) hard to keep enough volume to maintain skills, or a peds dedicated team 2) difficulty accessing training and consultation to manage patients' specific needs, 3) getting adult oriented providers to accept working with kids, both in terms of willingness and confidence, 4) billing issues: the traditional hospice benefit is not designed for kids, and can pose financial hardship for agencies if they are not careful on the front end, at time of referral, and negotiate a payment plan that works for them. We have had good success coaching and supporting these agencies, however, so these are not insurmountable problems. Hope that helps.
