Does anyone have a standard NICU template for EOL or PPC consultations you are willing to share?
Currently, our PPC team utilizes the 4 box model DMT (originating from Seattle Childrens) with our pediatric population. However, we have found the DMT too time consuming and the rapidly changing patient status/decision making process too difficult to keep up with in the NICU. Since our team has increased the amount of NICU consultations, we are looking to find or create a template that will provide documentation of the teams consultation for both the medical team and family. Any advice?

Hi,
Not to put Brian Carter at Vanderbilt on the spot, but he has published a protocol on NICU consultation that could easily be adapted to a protocol. And there are a number of programs that have begun in NICUs across the country, so perhaps some of them will share???!
