Hi. You are touching on a subject that has several important components to it. Firstly, there are the tools and templates that your team may choose to use to standardize care and direct your activities, and even establish standards of practice for your team and / or organization. Hopefully these will be based upon tried and true strategies for effective intervention, and you won't have to "reinvent the wheel." Many helpful tools are available through the CAPC PCLC-Peds process, including a trigger list for referrals, policy and procedure guideines, order sets examples,etc. from some well established programs. A second set of issues involves your team development process itself. A great strategy for developing "groupthink" and cohesion may in fact be to develop these practice guidlelines and the related tool box together, engaging interdisciplinary involvement to make decisions about the internal process of your program.
