Is anyone aware of a hospital utilizing pain and symptom management order sets or automatic consult triggers in pediatric palliative care?

At Children's Hospital and Clinics of MN as well as other pediatric palliative care centers, we are using order sets for continuous infusions with PCA of morphine, hydromorphone, fentanyl, midazolam, ketamine, propofol for pain and symptom management. In addition, the pediatric PCLCs have put compliled a "trigger" list as Dr. Friebert had referenced in an earlier post. We would love to use a trigger mechanism to initiate a palliative care "bundle" but our EMR cannot accommodate this. Does anyone have ideas of how to circumvent these problems without re-writing software programs for their EMR's?
