A colleague and I published a protocol a few years back in JPM on our approach to "Compassionate Extubation" and it is step by step protocol, it does describe option of sending the child home to do this as well but can be adapted for inpt only- intent was to make it more personal, more intimate and choreographed like a birth plan (which we had designed for perinatal hospice program.)
