Post Antenatal and perinatal palliative care

Do your patients deliver at your hospital, or do they deliver in the outlaying, island communities? Many patients travel long distances to see our specialists, then may have delivery, or induction in their small, community hospital. I am wondering about the best way to help them. Do you have any ideas? I am currently utilizing a birth plan format developed by Nancy English. Also, I am interested in the ways you approach a perinatal diagnosis with uncertain outcomes vs a sudden loss. The second is more like a trauma death and the first with more opportunity to prepare.
