Is anyone out there using a POLST type form for pediatrics? Also, any issues with local EMS not honoring Durable DNR forms?

CAPC is looking forward to the addition of two new programs as Palliative Care Leaderhip Centers. They will be rolled out over the next few months to be a resource for pediatric program development. The programs are: Akron (Ohio) Children’s Hospital and Children’s Hospitals and Clinics of Minnesota. At this time I don't have specifics for pediatric program development. Stay tuned. Aging with Dignity puts out a document called "My Wishes" which was designed for pediatric patients. It is based on the 5 Wishes document and is not legally binding, but it looks like a great tool. Go to http://www.agingwithdignity.org/mywishes.html for information about how to purchase this resource and to download a read-only copy. We have not used this form yet, and are an inpatient program, so I will not be able to speak to EMS, but I will post again once we get some experience with this or a similar form.

Have people had more experience with these issues as POLST is rolling out in more states? Our state form (Ohio) is being designed to be age-neutral, so it can be used with minors but there's nothing different about it. Just curious whether states who are ahead of us and have enacted POLST/MOLST already are running into problems?

Our hospital is just in the early stages of looking at adapting POLST, so don't have much to offer on that score yet. I can say that in our state,the honoring of DNR forms/ orders by community EMT providers is at their discretion, so that comes with some issues. We coach families about that, so that they understand that if they call 911 they may risk having interventions they would not like to have for their child. We try to communicate with EMS providers in advance when we can so they have a heads up on the context and hopefully agree to honor orders. We also talk with families extensively about options they have when their child stops breathing or has an event: call us, comfort their child(ren), etc. so that they feel some anticipatory planning is in place. Other experiences?
