Aging with Dignity puts out a document called "My Wishes" which was designed for pediatric patients. It is based on the 5 Wishes document and is not legally binding, but it looks like a great tool. Go to http://www.agingwithdignity.org/mywishes.html for information about how to purchase this resource and to download a read-only copy. We have not used this form yet, and are an inpatient program, so I will not be able to speak to EMS, but I will post again once we get some experience with this or a similar form.
