I am looking for sample EMR templates for palliative care consults and follow-ups in the outpatient setting. Would anyone have them and be willing to share?

I am not aware of any vendors that have EMR templates. Most of the programs that I am aware of have developed their own templates. It is recommended that you identify data points that you will be collecting and then have discussions with some IT resources to customize your template.
