Post Patient info for start/stop dialysis

We are drafting a patient handout to help them decide whether dialysis, for ESRD, will be beneficial for them; something which includes the various pros/cons, prognosis, etc. Does anyone have such a handout already which we could review and use as a guide if appropriate.

The National Kidney Foundation is a great place for info. They even have a great booklet "when stopping dialysis is your choice"
