While this looks like a good resource, it is by no means complete. Our hospital has had a palliative care program for four years, yet we are not listed on this website.

I will contact my network of palliative care social workers, and see if I can find any suggestions for you there.

If you are a hospital that provides a palliative care program, and you are not listed in the Palliative Care Directory of Hospitals or your information is incorrect, you should download an information form at: http://www.getpalliativecare.org/download/GPCPDIF1_final.pdf Complete the instructions on the form and return it to CAPC, ATT: CAPC RESEARCH. If your program meets the inclusion criteria listed on the form, it will be added or updated to the Palliative Care Directory of Hospitals. Thank you
