If you are a hospital that provides a palliative care program, and you are not listed in the Palliative Care Directory of Hospitals or your information is incorrect, you should download an information form at:

http://www.getpalliativecare.org/download/GPCPDIF1_final.pdf

Complete the instructions on the form and return it to CAPC, ATT: CAPC RESEARCH.

If your program meets the inclusion criteria listed on the form, it will be added or updated to the Palliative Care Directory of Hospitals.

Thank you - and I apologize, as our program actually is listed, just not under the correct city. So, I neglected to look at the entire Michigan list before posting. Thank you for the information.
