I've been doing home palliative care visits for about a year now and have some experience but am still learning, too. What specific questions do you have?

Dear Swissyolder,
I have been doing home visits for many years for patients living with life threatening illness. There are many practical suggestions I can give you to aid in your work. I would need a little more information about your position and the goals and objectives you are trying to achieve. For example, are these follow up visits after leaving the hospital after having been on an inpatient palliative care service? Are you functioning as physician or as an advanced practice nurse? Do you have a medical record system? Are you doing your own billing for services?

How are you going to be billing for Palliative Care home visits? Will it be through home health or hospice? Our palliative care program in strictly hospital based, I was not aware that you could bill for palliative care home visits. Would love to expand our services with some information on how to do so.

I am a physician for Kaiser Permanente who has been making home visits for our PC program since 2005. Our program is run as a home health benefit, and was developed in the late 90's at one of our So CA medical centers. The entire toolkit for putting together the program, including info on visit content, QI, etc., is available for download on the web at http://www.growthhouse.org/palliative/
In our case (a smaller area), we do not have "internal" Kaiser home health so we contract with a local home health agency and work closely with them, paying a per-visit rate. I'm happy to answer any questions I can.
