Post visiting physician palliative care service

Does anyone out there have a model for a visiting physician service dedicated to palliative medicine? My thought being a service where the physician would do home and nursing home/CBRF visits as a dedicated practice?

I think this would be very hard to sustain financially; it would not work in our system. We just cannot generate enough fee for service dollars to cover all our costs including salary, fringes, med school overhead, etc. We bill for the few home visits we make, but it does not cover the time to make them. Our House Calls program run by Peter Boling is not a break even program, either, to my knowledge. Sorry.

It could potentially work if you had a really low overhead and really low expectations for salary and wanted to work part time.
