The Fairview Palliative Home Care program provides palliative care to patients who qualify for home care and are appropriate for a "palliative" focus to their care. Home care guidelines are followed, and staff receive additional training in principles of palliative care with an emphasis on pain and symptom management, and communication about goals of care. About 50 % of these patients transfer to hospice prior to death. The remaining patients die in the palliative home care program. Their families receive the same bereavement follow-up as families whose loved one died in hospice.
The patients seen by the inpatient palliative care consult service are frequently followed by palliative home care which helps continue the plan of care outside the hospital or clinic setting.
