From a program development persepctive, aspects of a hospital based program may be applicable, as well as the approaches described in the CAPC/NHPCO/National Rural Health Asssociation's publication, Providing Hospice and Palliative Care in Rural and Frontier Areas. In Minnesota, the National Quality Forum's Preferred Practices became a framework for Minnesota's Rural Palliative Care Initiative ( see www.stratishealth.org).
Some home care agencies are able to develop a palliative care specific team to serve home care patients with palliative care needs. Hospice agencies are often key partners. Clinic case managers, especially those who work with people with chronic disease, may be able to incorporate palliative approaches.
Clinical eduation in palliative care for key clinicians through ELNEC or EPEC training and other self study programs are essential.
