Our outpatient palliative care program is currently finalizing a CHF program in conjunction with our Home Health division. We would be happy to share information we found very valuable. dblue@hospiceofwake.org

also interested in providing OP PC in established CHF clinic- we're already doing a few a month- but interested in expanding services- considering PC consult on all new low EF pts- early in trajectory. Anyone have experience in this area?
