We provide consults in 7 hospitals and do the very same thing. We dictate the initial consult and carbon copy our office. All the follow up visits are hand written and in the patent's chart at the hospitals. We have been able to get copies of any documentation needed from the hospitals and in fact recently went through a Cigna audit and we could produce all the required documentation. We do make a chart here in our office and keep the initial consult and document phone calls we may receive from patients or any other miscellaneous paperwork we have on the patient. We also have access to the hospital's system so we can print out demographic face sheets which we sent to billing.
