Post COPD Palliative Care Information

Was I not excited to find this forum. I am a Master's student at Calfornia State University, Fullerton working on my clinical hours for a hospice program. Part of my clinical time is for my change project towards trying to develop a COPD palliative care program for them developing algorithms / medications, palliative criteria, etc that can help guide a nurse that may be on call to assist the patient or family via telecommunication versus a visit to the ER. Being new to palliative care I am looking for some guidance if any one may be able to help with any documention, resources or suggestions. Thank you for your assitance.
