It might be helpful to start by defining what specific benefits you would like your residents and facility to realize from a more coordinated approach and what shortcomings you currently encounter. There are several successful service models in the nursing home setting - including hospice partnerships, palliative care consultation models linked with hospice partnerships, palliative care consultation models with outside partners and expanded nursing home service models. CAPC Palliative Care Leadership Center partner Palliative Care Center of the Bluegrass has experience working with nursing homes on this topic; Janet Braun at the Bluegrass site would be a good contact for you.
