Post patient centered care in Nursing homes

It makes sense that in order to provide quality care in nursing homes the physical design of the facility as well as the programming/philosophies of care can help enhance the provision of such care i.e palliative care/best friends approach in dementia patients. what are people doing in Oregon? I'm intersted in becoming part of a facility/nursing home that is looking at these issues.
